 When sending us repairs, please fill out the Repair Request form. This form is a PDF form file. 
  1. Open the Repair Request form in the Adobe Acrobat Reader - download here, if you need it, and directly type on the form. 
  2. When completed, save and print this form. 
  3. Include a copy of this form with your shipped unit for repairs.
